Dental Sealants: What They Are and What to Expect

What Are Dental Sealants?

Dental sealants are thin, protective coatings applied to the chewing surfaces of the back teeth — the molars and premolars — to shield them from decay. Made from a safe, tooth-colored resin material, sealants flow into the deep grooves and pits on the biting surfaces of these teeth and harden into a smooth, protective barrier that keeps bacteria and food particles from settling in and causing cavities.

The procedure is quick, completely painless, and requires no drilling or removal of tooth structure — making it one of the simplest and most effective preventive treatments available in dentistry. Sealants are most commonly associated with children, but they can be a valuable protective measure for teenagers and adults as well.

What Do Dental Sealants Treat and Prevent?

To appreciate why sealants are so effective, it helps to understand the unique vulnerability of the back teeth. The molars and premolars have highly irregular chewing surfaces covered with a complex landscape of hills, valleys, grooves, and pits — called fissures — that are designed to help grind and break down food. While this anatomy is excellent for chewing, it creates a significant challenge for oral hygiene.

Many of these grooves are extremely narrow — so narrow that the bristles of a toothbrush cannot physically reach the bottom of them. Food particles and bacteria collect in these crevices, protected from brushing and largely untouched by saliva’s natural cleansing action. Over time, the bacteria produce acids that attack the enamel at the base of these grooves, and cavities begin to form in areas that are virtually impossible to clean no matter how carefully a patient brushes.

Sealants address this vulnerability directly by filling in and sealing off these grooves before decay has a chance to take hold. Specifically, sealants help with:

  • Cavity prevention on molar and premolar surfaces: The chewing surfaces of the back teeth account for a disproportionately large share of all cavities — particularly in children and teenagers. Sealants reduce this risk dramatically by creating a smooth, sealed surface that is easy to clean and inhospitable to bacterial accumulation.
  • Protecting newly erupted permanent teeth: The years immediately following the eruption of permanent molars are the highest-risk period for pit and fissure decay. Applying sealants shortly after these teeth emerge provides protection during the most vulnerable window, before decay has any opportunity to begin.
  • Arresting very early decay: In some cases, sealants can be applied over the earliest stage of a cavity — where decay has just begun to affect the outermost layer of enamel but has not yet progressed into a full cavity requiring a filling. By sealing the area and cutting off the bacteria’s access to nutrients, the progression of the decay can be halted.
  • Reducing the need for future restorative treatment: Every cavity that is prevented by a sealant is a filling, crown, or more extensive treatment that never needs to happen. Sealants are a small upfront investment that can prevent significantly larger dental expenses down the road.
  • Protecting patients with deep grooves: Some patients — regardless of age — have naturally deeper or more complex groove anatomy than average, making them inherently more susceptible to pit and fissure decay. Sealants are particularly beneficial for these individuals.
  • Supporting patients with difficulty maintaining oral hygiene: Children who are still developing their brushing technique, patients with physical or cognitive limitations that make thorough oral hygiene challenging, and anyone who struggles to maintain consistent home care can benefit significantly from the added protection sealants provide.

Who Should Get Sealants?

While sealants are most commonly recommended for children, they are not exclusively a pediatric treatment. The following groups are the most likely candidates:

Children and Adolescents

The ideal time to apply sealants is shortly after the permanent molars erupt — typically around ages 6 to 7 for the first permanent molars and ages 11 to 13 for the second permanent molars. Applying sealants at this stage provides protection right from the start, before any decay has had the opportunity to develop. Many dental professionals also recommend sealing the premolars as they emerge during this same period.

Some children also benefit from sealants on their primary — baby — molars if those teeth have particularly deep grooves or if the child is at elevated risk of decay. Protecting primary teeth is important because they hold space for the incoming permanent teeth and play a critical role in chewing and speech development.

Teenagers

The teenage years bring increased independence, busier schedules, and dietary habits — frequent snacking, sugary beverages, and sometimes less consistent oral hygiene — that can elevate cavity risk. Teenagers who did not receive sealants in childhood, or whose existing sealants have worn away, may benefit from new or replacement sealants.

Adults

Adults without decay or existing restorations on their molar and premolar surfaces can also benefit from sealants. If the chewing surfaces of these teeth remain sound and sealant-free, applying them at any age adds a meaningful layer of protection. Adults with naturally deep grooves, a history of frequent cavities, or other factors that increase their decay risk are particularly good candidates.

How Are Dental Sealants Applied?

The sealant application process is one of the fastest and most comfortable procedures in all of dentistry. It requires no anesthesia, no drilling, and no removal of any tooth structure. The entire process for a single tooth takes only a few minutes, and multiple teeth can typically be sealed in a single appointment lasting 30 to 45 minutes.

Cleaning and Preparing the Tooth

The tooth to be sealed is thoroughly cleaned to remove any plaque, food debris, or surface staining from the chewing surface. This may be done with a rotating brush and cleaning paste, an air polisher, or hand instruments — depending on the hygienist’s or dentist’s preference and the condition of the tooth surface. The tooth is then rinsed and dried completely. A clean, dry surface is essential for the sealant to bond properly.

Etching the Tooth Surface

A mild acidic solution — called an etching gel or acid etch — is applied to the chewing surface of the tooth for approximately 15 to 30 seconds. This solution creates a microscopically rough texture on the enamel surface, similar to the way sandpaper roughens a smooth piece of wood. This texture is invisible to the naked eye but dramatically increases the surface area available for bonding, allowing the liquid sealant material to grip securely to the tooth. The etching gel is then rinsed off thoroughly and the tooth is dried again.

Applying the Sealant

The liquid sealant material is carefully flowed into the grooves and pits of the tooth using a small brush or applicator. The material is thin enough to penetrate deep into even the narrowest fissures, filling them completely. Your dentist or hygienist works carefully to ensure full and even coverage of all the groove anatomy on the chewing surface while keeping the sealant confined to the appropriate areas.

Curing the Sealant

Once the sealant is in place, a small handheld blue LED curing light is held over the tooth for approximately 20 to 30 seconds. The light triggers a chemical reaction that hardens the sealant from its liquid state into a tough, durable solid that is firmly bonded to the tooth surface. The transformation is nearly instant, and the tooth is ready to use immediately.

Checking the Bite and Finishing

After the sealant has hardened, your dentist or hygienist will check your bite to make sure the sealant does not interfere with how your upper and lower teeth come together. Because sealants add a very thin layer of material to the chewing surface, any high spots are identified and smoothed quickly and easily. A final inspection confirms that all grooves are fully covered and that the sealant is well adapted to the tooth surface.
What Can You Expect Afterward?

Sealants cause no discomfort whatsoever during or after the procedure. There is no recovery period, no sensitivity, and no dietary restriction beyond avoiding very hard or sticky foods for the first day or so to allow the sealant to fully set and stabilize. Patients can eat, drink, and go about their normal routine immediately following the appointment.

The sealant will feel smooth over the chewing surface and may initially feel slightly different to the tongue — this is completely normal and most patients adapt within a day or two without noticing it at all.

How Long Do Sealants Last?

With proper care, dental sealants can last anywhere from 20 years to a lifetime. They are durable enough to withstand the normal forces of chewing but are not indestructible — over time, they can chip, wear, or partially detach, particularly along the edges. This is why sealants are checked at every regular dental visit. If a sealant shows signs of wear or has partially come away from the tooth surface, it can be repaired or reapplied quickly and easily.

It is important to understand that a sealant that is fully intact provides complete protection to the sealed surface — but if a sealant chips or wears and is not repaired promptly, the groove beneath may become exposed and vulnerable again. Regular monitoring is therefore an important part of making sure sealants continue to do their job.

Caring for Sealed Teeth

Sealants are low-maintenance, but a few habits will help them last as long as possible and ensure your overall oral health remains strong:

  • Continue brushing twice daily with a fluoride toothpaste — sealants protect the grooves of the chewing surface, but the sides and gumline of every tooth still need regular attention
  • Floss once daily — sealants do not cover the spaces between teeth, where a significant proportion of cavities form
  • Avoid chewing on very hard objects such as ice, hard candy, or pen caps, which can crack or dislodge sealants prematurely
  • Avoid very sticky foods such as hard caramels or chewy candies that can pull at the sealant material
  • Attend regular dental checkups so your sealants can be examined and any wear or damage caught and repaired early
  • Continue receiving professional fluoride treatments as recommended — sealants and fluoride work together as a powerful team, with sealants protecting the grooves and fluoride strengthening the enamel on all other surfaces
