Root Canal Treatment: What It Is and What to Expect

What Is a Root Canal?

Root canal treatment — also called endodontic therapy — is a procedure that removes infected, inflamed, or damaged tissue from the inside of a tooth, cleans and disinfects the inner canal system, and seals it permanently to prevent reinfection. Rather than removing the tooth entirely, root canal treatment saves it — allowing a tooth that would otherwise need to be extracted to remain in the mouth, fully functional, for many years or even a lifetime.

The term “root canal” refers to the natural hollow spaces within a tooth that house the dental pulp — a soft tissue containing nerves, blood vessels, and connective tissue that nourishes the tooth during its development. When this pulp becomes infected or irreversibly inflamed, it can no longer heal on its own, and treatment is necessary to eliminate the infection, relieve pain, and preserve the tooth.

Root canal treatment is one of the most misunderstood procedures in dentistry. For decades it carried a reputation for being painful and dreaded — a reputation that modern techniques, advanced instruments, and effective anesthesia have made largely obsolete. The overwhelming majority of patients who undergo root canal treatment today report that the procedure was far more comfortable than they expected, and that the relief it provides from pre-existing pain and infection is immediate and significant.

What Does Root Canal Treatment Address?

Root canal treatment becomes necessary when the dental pulp — the living tissue inside the tooth — has become irreversibly damaged, infected, or has died. This can occur for a number of reasons:

  • Deep tooth decay: The most common cause of pulp involvement. When a cavity is left untreated for long enough, the decay progresses through the outer enamel and dentin layers of the tooth and eventually reaches the pulp chamber at the center. Once bacteria invade the pulp, infection sets in and the tissue can no longer recover on its own.
  • Dental abscess: A dental abscess is a pocket of infection — a collection of pus — that forms at the tip of the tooth root as a result of pulp infection spreading beyond the tooth itself into the surrounding bone. Abscesses cause significant pain, swelling, and in serious cases can spread to other areas of the head and neck. Root canal treatment eliminates the source of the abscess and allows the surrounding tissue to heal.
  • Cracked or fractured tooth: A crack that extends deep enough into the tooth to reach the pulp allows bacteria to enter and infect the inner tissue. Depending on the location and severity of the crack, root canal treatment may be needed to address the resulting pulp damage.
  • Traumatic injury: A blow to the tooth — from a fall, sports impact, or accident — can damage or sever the blood supply to the pulp even without visibly fracturing the tooth. In some cases the pulp dies gradually following trauma, a process that may not become apparent for months or even years after the original injury. A tooth that has darkened in color following trauma is often a sign that the pulp has died and treatment is needed.
  • Repeated dental procedures: A tooth that has undergone multiple restorations over the years — repeated fillings, replacements, and repairs — can experience cumulative irritation to the pulp that eventually leads to inflammation and failure. Each time a tooth is worked on, the pulp sustains some degree of stress, and for some teeth this accumulates to the point where the pulp can no longer recover.
  • Severe or prolonged inflammation: In some cases, significant pulp inflammation — called irreversible pulpitis — occurs without an obvious single cause. The pulp becomes chronically inflamed to the point where it cannot heal, and root canal treatment is needed to resolve the condition.

Signs and symptoms that may indicate root canal treatment is needed include:

  • Persistent, severe toothache — particularly throbbing or spontaneous pain that occurs without an obvious trigger
  • Prolonged sensitivity to heat or cold that lingers for more than a few seconds after the stimulus is removed
  • Pain when biting or chewing on the affected tooth
  • Darkening or discoloration of the tooth
  • Swelling, tenderness, or a raised bump on the gum near the affected tooth — sometimes called a dental fistula or “gum boil” — which is a sign that infection is draining from the root tip
  • A persistent bad taste or odor that does not resolve with normal oral hygiene

It is important to note that in some cases — particularly when the pulp has died completely — a tooth requiring root canal treatment may be entirely painless. This is why regular dental checkups and X-rays are essential for detecting pulp pathology even in the absence of symptoms.

What Happens Without Treatment?

A tooth with an infected or necrotic — dead — pulp will not heal on its own. Without treatment, the infection continues to spread — through the root tip into the surrounding bone, potentially forming or enlarging an abscess, destroying bone tissue, and in serious cases spreading to adjacent teeth or beyond. Dental infections that are left untreated can in rare but serious circumstances spread to the jaw, neck, or other areas of the head, becoming a significant medical emergency. Early treatment is always preferable — both for the health of the tooth and for your overall wellbeing.

Who Performs Root Canal Treatment?

Root canal treatment is performed by general dentists and by endodontists — specialists who have completed additional years of advanced training specifically focused on the diagnosis and treatment of pulp disease and root canal procedures. General dentists perform straightforward root canal treatments routinely, while more complex cases — such as teeth with unusual root anatomy, previously treated canals, or calcified canal systems — may be referred to an endodontist. Your dental team will determine the most appropriate provider for your specific situation.

How Is Root Canal Treatment Performed?

Root canal treatment is typically completed in one to two appointments depending on the complexity of the tooth and the severity of the infection. Teeth with more canals — such as molars, which may have three or four — or with more extensive infection may require a second visit to ensure complete healing before the tooth is sealed.

Before the Procedure — Diagnosis and Planning
Diagnosis begins with a thorough clinical examination and X-rays that allow your dentist to evaluate the condition of the pulp and the surrounding bone, assess the number and shape of the root canals, and identify the extent of any infection. In some cases a cone beam CT scan — a three-dimensional X-ray — may be taken to provide a more detailed view of complex root anatomy.

Your medical history and current medications will be reviewed, and the procedure, alternatives, and expected outcomes will be explained in detail so you can make a fully informed decision about your care.

Anesthesia and Comfort

Effective local anesthesia is administered before any part of the procedure begins. Contrary to the longstanding reputation of root canals as painful, the anesthesia used today is highly effective — and because the goal of the procedure is to eliminate the source of pain, most patients are pleasantly surprised by how comfortable the experience is.

For teeth with active infection or abscess, achieving full anesthesia can occasionally require additional anesthetic or a supplemental injection technique, as inflamed and infected tissue can sometimes reduce the effectiveness of standard local anesthesia. Your provider is experienced in managing this and will ensure you are thoroughly numb before proceeding.

Sedation options — including nitrous oxide or oral sedation — are available for anxious patients and can make the experience significantly more relaxed and manageable.

A rubber dam — a small sheet of thin latex or latex-free material — is placed around the tooth before treatment begins. This isolates the tooth from the rest of the mouth, keeps it clean and dry throughout the procedure, and prevents any of the irrigating solutions used during treatment from being swallowed. The rubber dam is a standard and important part of root canal treatment.

Access Opening

Using a dental handpiece, your dentist creates a small opening through the top — or back, in the case of a front tooth — of the tooth to gain access to the pulp chamber beneath. This opening provides the pathway through which all subsequent instrumentation and cleaning of the canal system is performed. The size of the access opening is carefully planned to be just large enough to allow thorough treatment while preserving as much healthy tooth structure as possible.

Locating the Canals

Using a combination of magnification — often provided by an operating microscope or dental loupes — and small, precise instruments, your provider locates and identifies each individual canal within the root system. The number of canals varies by tooth type — front teeth typically have one canal, while premolars may have one or two, and molars commonly have three or four. Identifying all canals is critical, as any untreated canal can harbor residual bacteria and lead to treatment failure.

Electronic apex locators — small devices that use electrical resistance to measure the precise length of each canal — are used to determine exactly how far each canal extends to the tip of the root. This measurement guides the depth of instrumentation and ensures thorough cleaning without overextending beyond the root tip.

Cleaning and Shaping

This is the heart of root canal treatment. Using a series of progressively larger, highly flexible nickel-titanium files — some rotated by a small handpiece and others used by hand — your provider carefully cleans the infected pulp tissue from each canal and shapes the canal walls into a smooth, tapering form that can be effectively filled and sealed.

Throughout this process, the canals are continuously irrigated with disinfecting solutions — most commonly sodium hypochlorite, which is a highly effective antimicrobial agent — that flush away debris, dissolve remaining organic tissue, and kill bacteria deep within the canal system. Additional irrigants such as EDTA — ethylenediaminetetraacetic acid — may be used to remove the microscopic layer of debris that forms on the canal walls during instrumentation, ensuring the cleanest possible surface for sealing.

The cleaning and shaping process is meticulous and methodical, working through each canal in sequence to ensure thorough disinfection of the entire root canal system.

Drying and Medicating (If a Second Appointment Is Needed)

If treatment is being completed in two visits — which is common when significant infection or abscess is present — the canals are thoroughly dried after cleaning and a temporary antimicrobial medication called calcium hydroxide is placed inside them. This medication continues to disinfect the canal system between appointments and helps reduce bacterial counts to the lowest possible level before the final seal is placed. A temporary filling is then placed in the access opening to keep the canals clean and protected until the second visit.

Obturation (Filling and Sealing the Canals)

Once the canals have been fully cleaned, shaped, and dried — either at the end of the first appointment or at the beginning of the second — they are permanently filled and sealed in a process called obturation.

The most widely used filling material is gutta-percha — a natural rubber-like material that has been used in endodontics for well over a century due to its excellent biocompatibility and dimensional stability. Gutta-percha is available in precisely sized, tapered points that correspond to the shape of the cleaned canals. The points are coated with a sealing cement and carefully compacted into each canal using a technique — such as warm vertical condensation — that ensures the material flows into all the complex internal spaces and creates a complete, three-dimensional seal.

The goal of obturation is to fill the canal system so completely and precisely that no space remains for bacteria to recolonize. A post-obturation X-ray is taken to confirm that the canals have been filled to their full working length and that the seal is complete.

Temporary or Permanent Coronal Seal

Once the canals are sealed, the access opening is closed with either a temporary filling — if a follow-up appointment is planned — or a permanent filling material. The coronal seal — the seal at the top of the tooth — is critically important, as research shows that recontamination of the canal system through a leaking coronal restoration is one of the most common causes of root canal failure. A well-placed, durable coronal seal is just as important as the root canal treatment itself.

After Root Canal Treatment — The Importance of a Crown

In the vast majority of cases, a tooth that has undergone root canal treatment will need to be protected with a dental crown following the procedure. This is a critical step that many patients underestimate in importance.

There are several reasons why a crown is typically necessary:

  • Structural protection: The access opening created during root canal treatment, combined with any pre-existing decay or prior restorations, leaves the tooth significantly weakened. Back teeth — molars and premolars — endure tremendous chewing forces, and an unprotected root canal-treated tooth is highly susceptible to fracture. A fracture that extends below the gumline can result in the loss of the tooth entirely — the very outcome the root canal treatment was designed to prevent.
  • Sealing against recontamination: A crown provides a durable, long-term seal over the access opening, protecting the treated canal system from bacterial recontamination.
  • Restoring function and appearance: A well-fitting crown restores the tooth to its full shape, size, and chewing function, and in the case of front teeth, its natural appearance.

Front teeth — which bear less chewing force — may sometimes be restored with a large filling rather than a crown, depending on how much natural tooth structure remains. Your dentist will assess the specific condition of your tooth and recommend the most appropriate restoration.

The crown is typically placed two to four weeks after root canal treatment, once any post-operative sensitivity has fully resolved and the tooth is confirmed to be healing well.

What Can You Expect After Root Canal Treatment?

Immediately After the Procedure
The local anesthesia will wear off over the course of one to three hours following the appointment. It is advisable to avoid eating until the numbness has fully resolved to prevent accidentally biting the cheek or tongue.

Post-Operative Discomfort
It is completely normal to experience some soreness, tenderness, or mild to moderate discomfort in the treated tooth and surrounding area for two to five days following the procedure. This is not a sign that something has gone wrong — it is a natural response of the surrounding tissue to the instrumentation and the resolution of the infection. The discomfort is typically well managed with over-the-counter pain relievers such as ibuprofen or acetaminophen, taken as directed. Prescription pain medication is occasionally provided for more significant post-operative discomfort.

Avoid chewing on the treated tooth until it has been restored with a permanent crown or filling, as the temporary restoration is not designed to withstand full chewing forces.

Healing and Follow-Up
A follow-up X-ray is typically taken six to twelve months after root canal treatment to confirm that the bone surrounding the root tip is healing normally and that no signs of persistent infection are present. In cases where a significant abscess was present before treatment, this healing can take a year or more — but in the vast majority of cases, complete resolution is seen on follow-up imaging.

Signs That Warrant a Call to Your Dentist
While some discomfort is expected and normal, the following signs should prompt you to contact your dental provider promptly:

  • Severe or worsening pain that is not controlled by over-the-counter medication
  • Significant swelling inside or outside the mouth
  • A return of symptoms after an initial period of improvement
  • The temporary restoration falling out
  • A visible bump or swelling on the gum near the treated tooth
Root canal treatment is not something to fear — it is something to be grateful for. It is a procedure that relieves pain, eliminates infection, and saves a tooth that would otherwise be lost. With modern techniques and anesthesia, it is a manageable and often surprisingly comfortable experience — and the result is a restored, healthy tooth that can serve you well for many years to come. How Successful Is Root Canal Treatment?

Root canal treatment has an excellent long-term success rate — studies consistently show success rates of 85 to 97 percent when the procedure is performed well and the tooth is properly restored with a crown afterward. A successfully treated and well-restored tooth can remain in function for decades — in many cases for the rest of the patient’s life.

In a small percentage of cases, root canal treatment may fail — typically due to recontamination of the canal system, a missed canal, persistent bacteria in complex canal anatomy, or a fracture of the root. When this occurs, several options are available:

  • Retreatment: The tooth is reopened, the original filling material is removed, and the canals are recleaned and resealed. Retreatment by an endodontist — who has access to advanced magnification and instrumentation — is successful in many cases.
  • Apicoectomy: A minor surgical procedure in which the very tip of the root is accessed through a small incision in the gum, the infected tissue at the root tip is removed, and a small filling is placed at the end of the root to seal it. This is typically considered when retreatment is not practical or has been unsuccessful.
  • Extraction: If neither retreatment nor surgical options are viable, extraction followed by tooth replacement with an implant or bridge may be the most appropriate course of action.
Caring for a Root Canal-Treated Tooth

A tooth that has undergone root canal treatment and been properly restored with a crown requires no special care beyond normal oral hygiene:

  • Brush twice daily with a fluoride toothpaste, paying careful attention to the gumline around the crown
  • Floss once daily, including around the crown margins where plaque accumulation can lead to decay at the gum line
  • Avoid chewing on extremely hard foods — ice, hard candy, or similarly hard items — which can crack even a crowned tooth
  • Wear a night guard if you grind your teeth, as grinding places excessive force on all teeth including those that have been crowned following root canal treatment
  • Attend regular dental checkups and cleanings so the crown and the underlying tooth can be monitored over time
  • Have follow-up X-rays taken as recommended so any changes in the bone around the root tip can be detected early
